Why this matters now

The global HIV response is navigating its most serious financing crisis in two decades. PEPFAR disruptions and USAID cuts in early 2025, combined with an estimated 6 million people in Africa remaining virally unsuppressed and progress toward HIV elimination still far from assured, have left ministries and implementing organisations urgently reassessing programme strategy and sustainability.

With direct experience inside PEPFAR, WHO, and UNAIDS during both programme growth and contraction, and active engagement in current policy debates, this consultancy provides technical advisory grounded in institutional knowledge and current evidence.

Reuben Granich is a physician-epidemiologist with more than 30 years of experience spanning clinical medicine, field epidemiology, applied research, and senior policy roles across national governments, the United Nations system, the U.S. government, academia, NGOs, and the private sector. His career has taken him to 32 countries across sub-Saharan Africa, South and Southeast Asia, Central and South America, and Eastern Europe.

Formal training includes a Doctor of Medicine from Stanford University School of Medicine, Internal Medicine residency at California Pacific Medical Center in San Francisco, a Master of Public Health in Epidemiology from UC Berkeley, Preventive Medicine and Public Health residency at CDC, and Epidemic Intelligence Service (EIS) training with the CDC Division of TB Elimination.

Malawi SmartNet Initiative

In partnership with PMI, the Malawi National Malaria Control Programme (NMCP), USAID, CDC, Dure Technologies, Chemonics, FHI360, and the University of Pennsylvania, led development of a pioneering community digital health network — one of the largest mobile health malaria programmes in Africa. In 2020, 300,000 long-lasting insecticidal nets (LLINs) were distributed across all 28 districts of Malawi via antenatal clinics. Each net carried a bilingual English and Chichewa label with a free-call short code, enabling recipients to join the network by mobile phone.

Over four surveys and a multi-channel media campaign from 2020 to 2022, the network grew to 124,167 unique respondents, covering an estimated 584,477 household members. Self-reported insecticide-treated net usage ranged from 59% to 83% across survey rounds, with highest usage in the rainy season. The PermaNet surveillance dashboard provided real-time monitoring of community survey density, ITN usage, malaria-like illness, and household net ownership across Malawi.

Behavioural economics trial (2021). A randomised trial among 61,009 SmartNet participants evaluated six behavioural economics-based health messages. The "bandwagon effect" message (7 percentage point increase in self-reported net usage, p<0.001) and "global slogan" message (5 percentage point increase, p=0.002) were the most effective — results with direct implications for malaria social and behaviour change communication at national scale. Presented at ASTMH Annual Meetings 2021 and 2022.

Media campaign (November–December 2021). A 30-day multi-channel media campaign in Kasungu District (population 885,624) using local radio, social media, and community events enrolled more than 25,000 unique respondents to the SmartNet network, demonstrating scalability to national level.
